What Is Root Resorption?
Root resorption occurs when the body begins to break down and absorb the root of a tooth. This process may result from trauma, orthodontic treatments such as braces, or advanced periodontal disease, characterized by the infection or inflammation of the gums and supporting bone. Root resorption can also result from impacted teeth pressing against the roots of adjacent teeth, cysts or tumors in the jawbone near a tooth, or constant teeth grinding and clenching, a condition known as bruxism.
There are two primary types of resorption: internal and external.
Internal Root Resorption: Internal root resorption begins within the tooth's inner pulp chamber or root canal system. This relatively rare condition is usually caused by chronic inflammation or a past traumatic injury to a tooth. As the internal tissues become overactive, they begin to dissolve the surrounding dentin from the inside out. On a dental X-ray, internal root resorption often appears as a smooth, ballooning expansion of the root canal space.
External Root Resorption: External resorption is much more common and begins on the outer surface of the root where the tooth meets the bone. It can be caused by different factors, such as impacted teeth pressing against neighboring teeth, teeth grinding and clenching (bruxism), or pressure from braces. Irregular edges on the outside of the affected tooth root are often telltale signs of external resorption.
Because resorption often causes little to no pain in its early stages, it frequently goes unnoticed and is usually discovered during routine imaging or during evaluations for other oral health concerns.
How Is Root Resorption Treated?
At Oak Creek Endodontics, we use CBCT imaging to diagnose root resorption, helping us identify the exact location, size, and type of resorption and determine whether the tooth can be effectively treated and saved.
Treatment depends on whether the resorption is internal or external and how advanced it is. While internal resorption is often treated with a non-surgical root canal to remove the overactive tissue within the tooth, external resorption may involve a minor surgical procedure to access the root surface. In these cases, we will create a small flap in the gum tissue to expose the affected root. Next, we will remove the diseased tissue, thoroughly clean the area, and place a bioceramic restorative material to seal the area and support healing. The last step is repositioning the gum tissue and suturing it to promote proper recovery.
If infection is present inside the affected tooth, we may also recommend root canal therapy as part of treatment. Internal or external root resorption repair can halt the resorption process and preserve the treated tooth, often preventing the need for extraction or replacement with a dental implant or bridge.
