Apexification for Immature Infected Teeth
San Antonio, Texas

When an immature permanent tooth has a deep cavity and infected roots, our experienced endodontist at Oak Creek Endodontics may recommend apexification. The endodontic procedure helps eliminate the infection, preserving a tooth that would otherwise require extraction. Saving a natural tooth is always our top priority and the best possible option for growing smiles.

What Is Apexification?

Apexification is an endodontic treatment designed to close or seal off an open root tip (apex) of an infected or damaged immature tooth. In immature, developing teeth, the apex is open and remains so while the tooth grows. An open apex can sometimes allow bacteria to enter the tooth's inner pulp and infect the nerve and soft tissue.

The procedure involves removing the infected pulp and placing medication in the root of an immature tooth. This helps create a hard barrier (apical closure) at the tip of the immature tooth root, allowing the root walls to thicken and strengthen. Unlike a procedure known as apexogenesis that helps preserve pulp vitality in healthy teeth, apexification is designed for infected teeth, helping prevent further inflammation and infection.

What Is the Apexification Process?

Before treatment, we ensure the area is fully numb using modern anesthetics. We are also happy to offer sedation options for patients who feel anxious or require additional support to relax during treatment.

During treatment, our endodontist will carefully remove any infected tissue from inside the tooth, similar to a root canal procedure. We use advanced irrigation techniques and ultrasonic devices to disinfect the root canal system thoroughly. Next, we will place medication and a bioceramic material to encourage the formation of a hard barrier at the root tip. This barrier will seal the open apex and allow the tooth to be restored and protected.

The modern bioceramic materials we use are highly biocompatible and support the healing of surrounding tissues, allowing for a one-step apexification process, unlike older methods, which required months of treatment and multiple visits. After apexification, your general dentist will create and place a dental crown to protect the treated tooth and restore its form and function.

When Is Apexification Recommended?

Apexification is typically recommended for children or adolescents with infected immature permanent teeth when regenerative endodontic treatment isn't possible. Left untreated, a tooth infection can lead to complications, including bone loss around the affected tooth, damage to the surrounding teeth, and even tooth loss.

Apexification may also be recommended for stabilizing an immature tooth with damage due to trauma, one with incomplete root formation, or one with roots eroded due to resorption. Closing the root tip also allows for successful root canal therapy when needed.

Why Choose an Endodontist?

It takes special skill and knowledge of a developing tooth's root anatomy to treat infected immature teeth. Endodontists have extensive training and are uniquely qualified to evaluate and treat the most complex endodontic cases, ensuring the best outcomes for your child's smile. With their extensive training and experience, as well as their use of modern diagnostic imaging, advanced endodontic techniques, high-powered microscopes, and ultrasonic instruments, you can always trust an experienced endodontist for your child or teen's endodontic care.
